Field Trip Signups & Digital Permission Slips

    July 14, 2026 · 9 min read

Paper permission slips are one of the last analog holdouts in most schools. Teachers send them home, half come back, some are missing signatures, and the office keeps a shoebox for the day of the trip. There's a better way — collect chaperones, drivers, allergies, and permission all in one online form that parents can fill out from their phone.

One Form, Everything the Trip Needs

Build a single form-mode project that includes every field the trip requires:

    • Student's full name
    • Grade and teacher
    • Parent/guardian email (required)
    • Emergency phone number (required)
    • Allergies or medical notes
    • "I give permission for my student to attend" (yes/no radio)
    • "I can chaperone" (yes/no)
    • "I can drive and have __ open seats" (number)

Every response lands in one dashboard. No shoebox, no missing signatures, and you can filter chaperones and drivers with one click.

    Digital Signatures Count

For most school districts, a parent completing an online form with their verified email address satisfies the permission-slip requirement. Check your district's policy first, but the vast majority now accept digital consent.

Send Reminders Until You Hit 100%

A week out, send a reminder to families who haven't submitted. Two days out, send another. The day before, send one last nudge with the emergency phone number for the day of the trip. Response rates go from 60% to 95%+.

Export the Roster for the Day Of

The morning of the trip, export the completed roster. You now have every student's parent contact, emergency phone, and allergy notes in one CSV — printable for the folder that rides the bus.

    Reuse Across Grades and Trips

Save the completed project as a template. Every future field trip is a five-minute duplicate: change the destination, date, and any trip-specific questions.
